TOLL Credit Request Form

PLEASE NOTE: The form below can be used to make toll credit requests for I-95 Express Lanes ONLY. The vehicle for which the toll credit requests is being made MUST be actively registered with South Florida Commuter Services with a 95 Express Low Emission(LEV)/Hybrid or 3+ Carpool vehicle decal. For toll credit requests/inquires for all other South Florida toll roads, please contact your transponder agency for information on how to file a dispute.
